For those referring physicians not using the electronic “Request for Holter Services” form

• Call us at 1.800.543.7275 x 203

• Give us the name of the ordering Physician,

• Phone number and

• The address of the practice, along with the group name, if applicable.

• Provide the patient’s name for which the Holter is intended,

• the date for which you would like the monitor delivered,

• and the name of whom you want to interpret the Holter Report.

We’ll deliver the Holter Monitor directly to you, along with instructions and supplies, and
Provide for the return of the monitor and the recording to us for prompt scanning and reporting.
A comprehensive report will follow, ready for you and your Interpreting Physician’s interpretation.
Affiliated Cardiologists are available to interpret Holter studies for those Physicians that wish to utilize other specialists to assist with their patients’ care.
